Management of the orthopedic oncology patient in pre/post surgery phase: integrated approach

The experience that we are carrying on a sample of 130 patients with “demolitive” surgical intervention affected by neoplastic muscle skeletal disease, specifically sarcomas, and their parents allowed a concrete multidisciplinary mode with a global take on responsibility, from the diagnostic comunication to dedicated pre/post surgery phases, in dismission, outpatient clinic and follow up. The emotional state, the experience of the disease the reactive mode the clinical evaluation of stress, anxiety and depression the psychological and emotional components of pain and arousal are important to assess. From the psychological assessment the following experiences emerged: anxiety related to fear of dying/diagnosis/surgery; anxiety compared to anesthesia and pain; negative thoughts; psychological distress/shame about the possible physical changes; fears and worries about loss of autonomy with difficulty in managing work and social-economic sphere, about family sphere with particular reference to loss of family role and the family’s emotions related to illness. About the pre/post surgical phases psychological problems such as mood disorder, depression, hyperarousal, anxiety, anguish, pain management difficulty arose. Preliminary results showed decrease of anxiety and arousal level, a better pain management and personal resource activation. Specifically, psychological intervention uses CBT with coping strategies, relaxation training, cognitive restructuration, problem solving, Biofeedback Training and EMDR. Psychological intervention with parents is effective to contain the psychoemotional aspects and to improve the communication for a better relationship with their beloved. Such multidisciplinary approach with psychological intervention promotes compliance to medical treatments, a better psychoemotional factors management and better functional personal resources activation.
